How does Live Oak work with my insurance?
We ask that you pay for your therapy at the time of service. On a monthly basis, we will bill your insurance company so that you may be reimbursed. Before your first appointment, it's important for you to check with your insurance company about any deductible you may have as well as any other benefit limits that relate to psychotherapy services.
Some of our therapists have a relationship with particular insurance companies, such as Private Healthcare Systems (PHCS) or APS Motorola, which would require you to be responsible for only a co-pay. We'll let you know if that option is available, but please feel free to ask your therapist about this before your first appointment
Does Live Oak offer a sliding scale?
Each of us at Live Oak has a fee range within which we operate. As much as possible, we will try to negotiate a fee that works for you.
Does Live Oak charge the same for couples and families as for individuals?
Yes, you will be charged the same fee regardless of whether you are seen individually or as a couple or family. There is usually a different fee if you participate in one of our groups.
How long does it take to see a therapist? Do you have a waiting list?
We respond to all requests or questions about appointments within one business day. We don't have a waiting list and can usually schedule an appointment within a few days of your call, depending on your schedule and any specific scheduling needs you may have.
Do you offer weekend and evening appointments?
Yes. Though many people request evening and weekend appointments, we will do what we can to accommodate your scheduling needs.
Live Oak is generally open during the following times:
Monday - Thursday: 8 AM - 10 PM Friday: 8 AM - 5 PM Saturday: 8 AM - 4 PM
Does Live Oak offer the services of a psychiatrist (M.D.) who can prescribe medication?
We don't have a psychiatrist on our staff, but we have relationships with many psychiatrists in Chicago and the surrounding suburbs. If it becomes necessary for you to see a psychiatrist while you are working with us, we will work with you to identify a doctor and will coordinate our services with him/her.
How does Live Oak communicate with my doctor, my other providers, and/or other people of my choice?
We believe it is important for all those with whom you are involved for care communicate and coordinate services. We will share information with others only after discussing this with you and only after you have signed an authorization for us to share information. You may also decide what type of information we will share and how frequently.

What forms of payment do you accept?
We are able to accept cash or checks.
Is there parking?
You may park in the parking lot directly west of the building after 5:30 PM on weekdays, and Saturday all day. At all other times, street parking is available.
Is Live Oak accessible by public transportation?
Yes. We are 5 blocks west of the "L" stop at Belmont (Brown, Red and Purple lines), and about 6 blocks south of the "L" stop at Southport (Brown line). The CTA #77-Belmont bus stops at the corner of Belmont and Lakewood, adjacent to our building.
Are the Live Oak offices physically accessible to all persons? (entering the building and once inside the building)
Yes. We have a wheelchair lift by the rear door of the building and a large, accessible elevator to reach our offices on the fourth floor. All bathrooms in the building are wheelchair accessible and meet ADA requirements. At the front of the building, there are five stairs from the sidewalk to the building entrance. If you have requests or questions about accessibility, please ask your therapist prior to your first appointment.
Can I bring a pet?
Building policy prohibits animals such as dogs or cats from being inside the building unless you rely on the animal for assistance.
Can I specify the gender or sexual orientation of the therapist I wish to see?
Yes. We have many qualified therapists and we will make every effort to connect you with a therapist who can best meet your needs
What should I do prior to my first appointment?
Before arriving for your first appointment, it may be helpful for you to do a few things:
First, if you plan on using insurance, please make contact with your insurance company to be sure you understand your benefits, deductible, and/or co-pay. Please also bring your insurance card with you.
Second, you may choose to familiarize yourself with our general thoughts and policies about therapy. You may download our "Statement of Agreement" and print it for your reference, though your therapist will also review it with you during your first session.

Third, you may download our "Privacy Policies" for your information. Your therapist will also provide you with a copy of these policies at your first session.

Fourth and last, take a moment to think about what questions you might have for your therapist during your first session. It may also be helpful to think about what you would like to accomplish. Please remember that you do not have to tell your therapist "everything" during your first session. Rather, it is an opportunity to share enough to determine the goodness of fit between you and your therapist. As you continue in therapy, you will undoubtedly clarify your goals.
